  A referral is required from your optometrist or general practitioner for your appointment with one of our specialist ophthalmologists. That referral enables you to claim a rebate from Medicare on your consultation fee.

  We necessarily need to know relevant details of your medical history prior to admitting you to Vision Centre Day Surgery for your eye procedure.

  Our Pre-Anaesthetic Questions form is available here for downloading and printing. Please ask your local doctor to complete this form prior to your surgery. The completed form should then be:
